TESTING/EVALUATION

Common evaluations and their components are listed below. Please contact Alex directly if you or your child are seeking an evaluation not included in the list below.

All evaluations follow the same fee structure:

  • $300 (flat fee) - 60-120+ minute plus initial intake session for diagnostic interview and developmental history

  • $275 per hour of direct face-to-face testing - no additional cost for time spent reviewing records, scoring and interpreting test results, and report writing

  • $300 (flat fee) - 60-120+ minute feedback session following completion of testing to review results, discuss recommendations, answer questions, and coordinate care


An itemized receipt of services (‘Super Bill’) is provided upon request for reimbursement via your insurance plan’s out-of-network (OON) benefits.

During your free consultation call, you partner with Alex to identify what evaluation is most appropriate for you or your child. A fee estimate is also provided at that time.

Feedback sessions to review test results and discuss recommendations are usually scheduled 7-10 days following completion of testing.

ADHD Evaluation

Two (2) to four (4) hours of testing on one day

Can include any/all of the following:

  • Review of educational and/or medical records

  • Initial intake session for diagnostic interview and developmental history

  • Cognitive/intellectual abilities (IQ)

  • Academic skills (reading, math, writing)

  • Memory, attention, and processing speed

  • Emotional, behavioral, and/or adaptive assessment (self-, parent-, and/or teacher-report rating forms)

  • Personality assessment (for adolescents and young adults)

  • Feedback session to review testing results and recommendations

Academic/Learning Evaluation (Psychoeducational)

Two (2) to four (4) hours of testing across one or two days

Can include any/all of the following:

  • Review of educational and/or medical records

  • Initial intake session for diagnostic interview and developmental history

  • Cognitive/intellectual abilities (IQ)

  • Executive functioning (i.e., visual/auditory memory, learning, processing speed, attention)

  • Academic skills (reading, math, writing)

  • Phonological and orthographic processing

  • Emotional, behavioral, and/or adaptive assessment (self-, parent-, and/or teacher-report rating forms)

  • Personality assessment (for adolescents and young adults)

  • Feedback session to review testing results and recommendations

Mental Health Evaluation (Treatment Planning)

Two (2) to four (4) hours of testing across one or two days

Can include any/all of the following:

  • Review of educational, medical, and/or prior psychological/psychiatric records

  • Collaboration with current therapist, psychologist, and/or psychiatrist (if applicable)

  • Initial intake session for diagnostic interview and developmental history

  • Cognitive/intellectual abilities (IQ)

  • Executive functioning (i.e., visual/auditory memory, learning, processing speed, attention)

  • Brief academic skills (reading, math, writing)

  • Emotional, behavioral, and/or adaptive assessment (self-, parent-, and/or teacher-report rating forms)

  • Personality assessment (for adolescents and young adults)

  • Feedback session to review testing results and recommendations

Gifted Evaluation

Two (2) to three (3) hours of testing on one day

Can include any/all of the following:

  • Review of educational and/or medical records

  • Initial intake session for diagnostic interview and developmental history

  • Cognitive/intellectual abilities (IQ)

  • Academic skills (reading, math, writing)

  • Feedback session to review testing results and recommendations

Intellectual Disability (ID) Evaluation

Two (2) to three (3) hours of testing on one day

Can include any/all of the following:

  • Review of educational and/or medical records

  • Initial intake session for diagnostic interview and developmental history

  • Cognitive/intellectual abilities (IQ)

  • Academic skills (reading, math, writing)

  • Emotional, behavioral, and/or adaptive assessment (self-, parent-, and/or teacher-report rating forms)

  • Feedback session to review testing results and recommendations


THERAPY & COLLEGE PLANNING

Psychotherapy (60 minutes)

$175 per session

One-time initial intake session ($300); individual 60-minute therapy session ($175 per session)

College Planning & Preparation (60 minutes)

$150 per session

Can include any/all of the following:

  • One-on-one meetings with your child to discuss academic performance and extracurricular involvement

  • Guidance and recommendations on course/class scheduling during high school

  • Support for test preparation, Common App completion, review of personal statement and essays, resume building, and academic accommodation eligibility

  • Research and guidance on college choices and preparation

  • Assistance in scheduling deadlines/timelines for college choice decision-making, application submission, and college visitation

Out-of-Network (OON) Provider

Alex is an ‘out-of-network’ (OON) fee-for-service provider. He is not contracted or paneled with insurance carriers or Medicaid. However, upon request, he provides you with the necessary and appropriate documentation to submit for possible reimbursement via your OON benefits (unless you have Medicare) This documentation is often referred to as a ‘Super Bill’ (receipt of services).

Please check with your insurance carrier to learn more about your OON mental health benefits. When speaking with insurance, consider:

What mental health benefits does my plan offer?

Are certain mental health diagnoses excluded from my plan’s coverage?

How much does my plan reimburse for OON services?

How many sessions does my plan cover?

Is pre-authorization, referral, or approval from my pediatrician or family physician required?

Why private pay?

Insurance companies are profit-driven, not client-centered. They enact limitations on the amount of time Alex can spend with you and/or your family. They also often attempt to dictate the way services are provided regardless of what may be best for you or your child’s unique circumstance.

Alex does not accept insurance so he can spend more time with you in providing the specialized services best for you or your family.

Payment

Alex accepts cash, credit/debit, and HSA/FSA payments for services. Payment is due prior to or at the time of service, and accepted in-person (cash) or through the secure payment portal (credit/debit). If paying via credit/debit or HSA/FSA, a card remains on file in our payment portal for timely processing of payments at the time of service. Payment plans, discounted fees, and pro bono services are considered on an individual basis.
